This form is intended as documentation for reporting any volunteer, coach, player, or official whose actions are suspect of having violated the code of ethics. 2. Incidents must be submitted on this standard form. 3. These printed forms are available from your team coach/manager, Scorers table, Headquarters tent, hospitality room, Tournament Director, and on the NBBA website. 4. The form should be completed with as much detailed information as possible. 5. The completed form should be submitted as soon as possible or within up to 24 hours of the incident to the Ethics Committee. ethics@nbba.org 6. Upon receipt, the committee will review the form and respond to you acknowledging receipt of the form with next step (if applicable). 7.
